Abstract
서론
악관절 강직증이란 개구장애, 전방 및 측방운동의 제한등과 같은 악골의 운동성장애를 일
컫는 것으로 Kazanjian 등이 처음으로 진성강직과 가성강직으로 분류하였고 이를 다시 편측
성 및 양측성으로 분류하였다. 이는 또한 관절내 강직과 관절외강직으로 분류되기도 하며,
진성강직은 섬유성강직과 골성강직으로 다시 분류된다.
관절내강직 또는 진성강직증의 원인으로는 외상, 감염, 전이성 암종, 방사선조사, 화상이나
술후합병증등에 의해 발생된다고 하였다. 또한 악관절 강직증이 성장기에 발생하게 되면 기
능적인 장애뿐만 아니라 하악의 성장과 위치에도 영향을 미치게 되어 안면비대칭 하악왜소
증등의 안모불균형 및 부정교합등과 같은 문제등을 야기하게 된다. 이러한 악관절 강직증에
대한 외과적 술식으로는 융합된 과두돌을 절제하는 과두절제술, 융합된 과두돌기와 상행지
사이에 공간을 형성해주는 간극성 악관절성형술 (gap arthroplasty), 절단골면사이에 이식물
질을 위치시키는 개재성 악관절 성형술(interpositional arthroplasty), 골절제후 골이식 혹은
인공적인 보철물을 통한 악관절재건성형술등의 다양한 수술법들이 제시되어왔다. 이에 저자
등은 본교실에서 시행한 악관절 강직증의 증례중 진성강직증을 중심으로 각각의 원인 및 수
술방법, 그리고 술후의 정기적 재내원을 통한 개구량 및 합병증여무등을 연구조사하여 문헌
고찰과 함께 보고하는 바이다.
TMJ ankylosis is defined as a mobile disorder of jaw such as mouth opening
limitation, limitation of anterior or/and lateral movement of TMJ. Kazanjian published
first clinical report about classification of TMJ ankylosis dividing with intracapsular
ankylosis and extracapsular ankylosis. TMJ ankylosis is resulted from trauma, infection,
metastatic tumor, irradiation, burn and etc. When TMJ ankylosis is manifested in
growing period, it affects to functional disorder and development and position of
mandible, so it can result in maxillofacial deformity such as facial asymmetry,
micrognathia, malocclusion. For treatment of TMJ ankylosis, various surgical
interventions were devised ; condylectomy, gap arthroplasty, interpositional arthroplasty
and TMJ reconstruction. So, we report our results with documental study and cases of
true ankylosis in our department.
